State disciplines health care providers (03-26-2026)

Olympia -- The Washington State Department of Health has taken disciplinary actions or withdrawn charges against the following health care providers in our state.

The department’s Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nursing assistants, counselors).

Information about health care providers is on the agency website. View the “Health Care Provider Lookup” under the “Find it Fast” section of the Department of Health website (doh.wa.gov).The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are encouraged to call and report their complaint.

Grays Harbor

In February 2026 the Department of Health charged home care aide Desaraye Lucille Faye Whited (HM61556309) with unprofessional conduct. In February 2025, Whited allegedly gave a patient medication that was not prescribed and that Whited was not authorized to provide.

King County

In February 2026 the Department of Health charged midwife Rhonda Shannon Mitchell (MW60861142) with unprofessional conduct. Beginning in August 2020, Mitchell allegedly failed to address a patient’s risk factors during pregnancy. In April 2021, the patient’s fetus died during childbirth. After delivery, Mitchell allegedly failed to appropriately respond to possible complications and continued treating the patient without consulting or transferring care to a physician.

In March 2026 the Board of Osteopathic Medicine and Surgery charged osteopathic physician Rachael Murphy (OP61251834) with unprofessional conduct. In March 2025, Murphy allegedly improperly prescribed medication to a patient. Murphy did not respond to the department’s requests for information.

In March 2026 the Department of Health charged certified medical assistant Larry Vern Smalley (CM60901639) with unprofessional conduct. Between June and August 2024, Smalley allegedly violated a protection order multiple times by contacting a person and going to the person’s home without permission. In July 2024, Smalley was charged with criminal trespass.

Pend Oreille County

In February 2026 the Department of Health withdrew its statement of charges against Grady Clayton Hoaglund (LH61118105, MC60917920), a mental health counselor associate and mental health counselor, because Hoagland surrendered his credentials by agreement with the Department.

Pierce County

In March 2026 the Department of Health charged substance use disorder professional trainee Katrina Hanson (CO61457400) with unprofessional conduct. Between June 2023 and April 2024, Hanson allegedly provided clinical services to patients without approved supervision.

In February 2026 the Department of Health denied Monica Nganga’s (HM61579416) application for a home care aide credential. Between March and May 2024, Nganga allegedly engaged in a romantic relationship with a client, accepted gifts and cash from the client, and used the client’s credit card to pay for personal travel.

Snohomish County

In March 2026 the Chiropractic Quality Assurance Commission terminated the probation on Kyle Matthew Osborne’s (CH60450085) chiropractic credential. 

Spokane County

In March 2026 the Pharmacy Quality Assurance Commission granted Paige Boisen’s (VB70047794, VA61676619) pharmacy technician and pharmacy assistant credentials subject to conditions. Between March 2025 and August 2025, Boisen worked as a pharmacy technician without an active credential.

Thurston County

In March 2026 the Department of Health charged certified nursing assistant Linda D. Gailey (NC10067972) with unprofessional conduct. In September 2024, Gailey was reported as allegedly unable to practice with reasonable skill and safety due to a medical condition. Gailey did not respond to the department’s request for information.

In February 2026 the Department of Health withdrew its statement of charges against David C. Simonsen (LF00001940), a marriage and family therapist.

Yakima County

In March 2026 the Department of Health charged agency affiliated counselor Raeann Elaine Rowe (CG60536869) with unprofessional conduct. From 2023 through 2024, Rowe allegedly accessed a client’s records multiple times without a legitimate reason. In May 2024, the client reported that Rowe accessed and shared the client’s confidential treatment information without consent.

Out of State 

Florida: In November 2025 the Pharmacy Quality Assurance Commission and Pharmcore, Inc. doing business as Hallandale Pharmacy (FO60922880) entered an agreed order in lieu of suspension, revocation or modification of Pharmcore’s license. In 2018, Pharmcore changed its Florida location and failed to notify the commission within the required 90 days. Pharmcore must comply with the order’s terms and conditions, including paying a $10,000 fine within 12 months.

Texas: In March 2026 the Pharmacy Quality Assurance Commission charged Echo Health, LLC doing business as Nova Specialty Pharmacy with unlicensed practice and notified the business of its intent to issue a cease-and-desist order. Between January and August 2024, Nova Specialty Pharmacy allegedly shipped drugs to patients in Washington without holding a Washington pharmacy or nonresident pharmacy license.


Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
